You will need to request your transcript in writing, either by mail or fax. List your name (at time of testing), date of birth, Social Security Number and the approximate year you tested. You will also need to list either the address or the fax number where you want the transcript sent. Please include your signature. Request the transcript by fax at (715) 365-4542 or by mail at P.O. Box 518, Rhinelander, WI 54501, Attention: Lori Vance.
